Anxiety Self- Monitoring Record

This is a feedback activity designed for recording Anxiety self-monitoring. This should be completed during or shortly after an event.

Self-monitoring for anxiety is observing and recording the specific targets such as the situation, body feelings, emotions and behaviors. Self-monitoring is a technique that is often introduced early in the therapy process and can continue to provide helpful insights throughout treatment and recovery. The goal is to become more aware of our experiences and the contexts in which they occur. The Self-Monitoring Record activity is designed to help you capture information about situations where you experienced feelings of fear or anxiety. The activity includes opportunities to record information in the following contexts: situational context; the content of anxious predictions, emotional and physiological reactions; and coping responses; and consequences of these coping responses.
